Understanding the problem

What is spinal stenosis?

With age, the discs lose height, the ligaments thicken and the joints grow small bone spurs. Together, these narrow the space for the nerves in the lower back (lumbar canal stenosis).

The squeezed nerves cause pain, tingling and heaviness in the legs when you walk or stand. Sitting or bending forward opens the canal a little, so the pain eases. Many patients can walk only a short distance before they must sit down.

Common causes

  • Age-related disc and joint wear
  • Thickened ligaments in the spine
  • Bone spurs from arthritis
  • Slipped vertebra (spondylolisthesis)
  • Old injury or earlier spine surgery

Treatment, step by step

How Dr. Nitish treats spinal stenosis

The simplest treatment that can work comes first. Surgery is advised only when it is really needed.

Step 1

Medicines and activity

Nerve pain medicines and walking with short rests.

Step 2

Physiotherapy

Flexion exercises, core strength and cycling, which is often easier than walking.

Step 3

Epidural injection

A caudal epidural or nerve root block to ease the leg pain.

Step 4

Decompression surgery

Removes the bone and ligament pressing the nerves, with fixation if the spine is unstable.

Is spinal stenosis a serious problem?

It is not life-threatening, but it can slowly limit how far you can walk. Treatment helps you stay active and independent.

Can spinal stenosis be treated without surgery?

Yes, mild to moderate stenosis is often managed with medicines, physiotherapy and injections. Surgery helps when walking becomes very limited.

What is the success rate of stenosis surgery?

Decompression surgery relieves leg pain and improves walking in most well-chosen patients. Dr. Nitish explains what you can expect based on your MRI.

Is surgery safe in elderly patients?

Yes, with proper checks. Many patients in their 60s and 70s have stenosis surgery and walk better afterwards.
